Job summary
An exciting opportunity has arisen within the Onward Care Team for a Discharge Co-ordinator.
You will play an important role in supporting the clinical staff in facilitating safe and timely discharges from hospital. Helping to provide a seamless service for patients currently admitted to Worcester Acute Hospitals. This will encompass cross boundary and collaborative working within a multi-disciplinary team.
This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills, working knowledge of the discharge process and possess excellent communication and organizational skills. Ideally having previous experience of working within a clinical setting.
This is a 37.5-hour full-time position working over a 7-day week based at the Worcester Royal Hospital to include cross site cover to the Alexandra Hospital site inclusive of bank holidays.

Main duties of the job
The aim of the role is to ensure that all patients are tracked through their ward stay, to ensure timely discharge including highlighting any escalations where necessary.
The post holder will have awareness of and escalate delays along a patient's journey, working with the Onward Care Team.
Actions the post holder will implement to reduce delays will include liaising directly with the clinical staff on the ward, liaising with specialties and speaking with patients and relatives.
The post holder will work proactively, pre-empting delays where possible, and helping to plan for discharge.
