We have a vacancy in our team
for a Mental Health Community Practitioner. We work in a population based
mental health care for the community of Worcester. Working closely with our
partners in the Voluntary Care Sector and multi-disciplinary
teams of highly specialist practitioners to provide care and treatment. We are building on our links with
our Primary
Care Networks to help support the patient journey. If you are seeking a role
that will encompass new ways of working that will be the future delivery of
community mental health care in England, then this is an opportunity you should
not miss. The post is for 37.5 hours a week (Monday to Friday), although we
would consider applications for 22.5 hours a week over 3 days. We also allow
flexibility for staff to work from home where clinically possible and
compressed hours.
Main duties of the job
The main
duties of a Mental Health Community Practitioner within our team includes (but
not limited to):
- Completing assessments and triages with patients referred
within the Worcester locality. Referrals would typically come from GPs, however
may also come from internally within the trust or other agencies.
- Holding a caseload of patients with a providing short
term psychological treatment to these individuals using evidence-based
interventions such as CBT, Solution focused, mindfulness.
- To provide group interventions and 1:1 focused treatment.
- To be able to risk assess and risk manage patients in
response to their current needs.
- To take part in the duty system and act appropriately
when dealing with patients who are experiencing difficulties in the absence of
their allocated worker.
