Job summary
An exciting and rare opportunity has arisen for the post of a Band 5 Qualified Nurse for the Learning Disability Replacement Care Service at Churchview, Bromsgrove. This will be a 12 month fixed term contract/secondment to cover maternity leave.
We are looking for a dynamic and motivated Nurse to assist the Replacement Care Nursing team to deliver a clinically effective service with the overarching aim that the adults who attend the service feel welcome, safe, confident, and keen to stay, thus providing a positive experience for all concerned. We are a very welcoming team and happy to support a newly qualified nurse within this brilliant role.
The successful applicant will be covering the duties of a Qualified Nurse during the permanent Band 6’s maternity leave; this involves shift work (07.00-14.30, 14.00-21.30 and 21.15-07.15) and 37.5 hours over 5 days.

Main duties of the job
The role includes admitting and discharging service users, their belongings, and their medications for their over night stay. Regular communications with families and other professionals involved in their care such as day centre staff, college staff, GP & Community Learning Disability Nurses. You will deliver the highest standards and quality outcomes possible, ensuring that the service user is at the heart of your care and decisions.
You will be required to support service users with personal care tasks and moving and handling aspects of care alongside medicines management. We are a supportive, passionate, and motivated staff team and reflect on the care we provide regularly to improve the care we provide to service users. The candidate will be supported to supervise a 3-4 Health Care Assistants and will need the confidence to lead a shift and delegate duties. We deliver person centred care which involves regular reviews with families.
You will utilise carenotes, feedback to families and handover to the team. Knowledge of developing and evaluating care plans for evolving health needs of the service users, ensuring changes are documented and reflect the service users needs. We complete Pre-admission Assessments for new service users to confirm eligibility to the service and to develop care plans to encourage a smooth transition into the service. Develop Positive behaviour Support plans to encourage a successful stay.
