Cityside Neighbourhood Team is a friendly
community team who are looking for an enthusiastic individual to come and join
us in supporting patients in their own homes.
We are looking to recruit a Nursing
Associate to work on a part-time or full-time basis. We would welcome a Nurse who is keen to join the team and enhance their
existing skills.
We work closely with our 5 GP
surgeries and maintain effective relationships with other primary care services
and together we are an extremely caring team who are all committed to
developing our services to provide the most personalised care for our patients.

Main duties of the job
The role will require:
- The ability to communicate effectively with others at all levels
- Working flexibly to provide the service delivery need across the local area (the service operates from 7:30am – 6:30pm, Monday to Sunday including bank holidays)
- An enthusiastic, motivated and innovative thinker to become part of a primary care facing community
- The ability to problem solve and manage time effectively
- Experience of working in a fast paced and varied environment
- The ability to travel to deliver care services
Your typical working day will include home visits for adults over 18 years needing a range of nursing interventions. These include caring for people reaching the end of their life, all types of wound care- post operative wounds, leg ulcers, pressure ulcers and dressing regimes. You will work from a mobile tablet where you can update your records as you visit each patient, the team then meet at base for a handover and to review.

management, continence care, palliative care and medicine management. This
involves planned care and unplanned care.
As a community associate nurse, you will have the support of experienced
clinicians to develop your skills in a supportive environment and be actively
involved in ongoing planning of patients care. There will be the opportunity,
an identified part of your role, to support students and community healthcare
assistants with their ongoing learning .
For purposes of the role you must be NMC registered, associate nurse (or
about to qualify ) hold a full UK driving license, have good IT skills and have
excellent communication skills.
