Job summary
We are looking for an experienced transformation leader who can turn strategy into delivery, build strong partnerships, and lead largescale change that improves outcomes for patients and staff. This is an exciting opportunity for a dynamic, values driven Head of Transformation & Integration to join our Operational & Transformation Senior Management Team.
This is a high impact leadership role at the heart of our Trusts ambition to deliver integrated, digitally enabled and financially sustainable services. You will provide senior, visible leadership across a diverse portfolio, shaping the future of care pathways and ensuring our services meet the needs of our communities now and in the future.
This is a unique opportunity to help shape a developing transformation function and make a meaningful impact on how we deliver services. You will work alongside supportive senior leaders, have the freedom to innovate, and play a key role in driving modernisation, efficiency and improved outcomes for staff and service users.

Main duties of the job
As Head of Transformation & Integration, you will:
Provide senior leadership for divisional transformation, turning strategy into delivery through disciplined programme management and robust benefits realisation.
Lead a portfolio of transformation, integration and pathway redesign programmes, ensuring they draw on national best practice, local context and patient and staff feedback.
Translate Trust strategy into 1, 3 and 5year delivery plans, aligned to organisational and system priorities.
Act as SRO/Programme Lead for major programmes, ensuring governance, risk, benefits and change control are rigorous and transparent.
Provide operational leadership across services, ensuring safe, effective and patientcentred delivery.
Build strong, positive partnerships with commissioners, system partners and local organisations to support integrated pathways of care.
Lead and develop the Transformation & Integration Team, fostering a culture of improvement, compassion and high performance.
Ensure service users, carers and families are consistently involved in shaping and evaluating services.
Deputise for the Associate Director of Operations & Transformation when required.
