We are looking for an enthusiastic SCPHN to join our team on 30 hours per week. As a SCPHN School Nurse you will be responsible for delivering a core service in accordance with the Healthy Child Programme to improve health outcomes for school aged children and young people (5-19 years). School Nurses work closely with education and health, providing a link between school, home and the community. You will drive forward the public health agenda through collaborative and innovative working.
You will be joining friendly and supportive 0-19 Public Health Nursing teams.
The team cover both rural and urban areas and being able to meet the travel requirements of the role is essential. Travel across the locality, and Worcestershire, will be required.

Main duties of the job
As a SCPHN School Nurse you will lead a skill mix team to deliver the Healthy Child programme and High Impact Area work, to meet the diverse health needs of the population which includes the delivery of screening health questionnaires, the triaging of these and then implementing support as required. Referring and liaising as necessary. Through monitoring outcomes using evidence-based tools you will strive to improve service delivery through innovative practice. Multi-agency working is key to this role, and you will need to be confident working with other professionals to empower the most vulnerable families to make positive change. You will be confident in your decision making and report writing.
