Main duties of the job

As a Senior community nurse working with the Evening and night Team most of the work allocated to you will of an unplanned nature. The service works as an on-call service covering the County of Worcestershire taking direct referrals from patients, 111, Ambulance Service and discharging hospitals. You can expect to have a comprehensive induction and work along side peers until you are confident to then work alone.

The evening service nurses primarily work alone, they have work allocated via a handheld tablet by the triage nurse based at Droitwich. You will be able to start your visits from home if the geographics dictates.

The patients that are seen during the evening and night vary immensely and no two shifts are the same. For example, we see catheters, palliative care, wound care, administration of medication, diabetics, and patients requiring intensive support rehabilitation following discharge from hospital or admission prevention.

As part of a new initiative, we are working closely with West Midlands ambulance service and 111 and assist in responding to urgent Care response. These can be patients that have fallen, or patients requiring further assessment to aid prevention in admission to hospital.
