Job summary

This is a part time post working 31 hours per week. However, we would consider flexible working and less hours for the successful candidate but would have to be a minimum of 30 hours.

Main duties of the job

As a member of the reablement team, you will work closely with health and social care staff, having the unique opportunity to experience both sectors.

You will work under the guidance of qualified therapists, providing patient centred interventions, and following treatment programmes to meet the goals of each service user. You will hold a small caseload of simple cases, undertaking regular reviews of their progress and altering their reablement plan accordingly to maintain a goal focus.

You will work closely with both health and social care colleagues, in a blended approach to patient care. Physiotherapists, nurses, occupational therapists, therapy assistants and social care staff undertake collaborative working on a daily basis to ensure that all patient needs are met. Regular clinical supervision and professional support are provided.
