Job summary
An exciting opportunity has arisen to employ a qualified Nurse associate
to our Hereford City Neighborhood team. This role will focus on the delivery of
population based mental health care for the community of Hereford. You will be
working alongside multi-disciplinary teams of highly specialist practitioners
and have direct relationships with the professionals within the local Primary
Care Networks (PCN). If you are seeking a role that will encompass new ways of
working that will be the future delivery of community mental health care in
England, then this is an opportunity you should not miss. The post is for 37.5 hours
a week (Monday to Friday).
Main duties of the job
The main duties
for a Nursing Associate within our team includes (but not limited to):
- Administering
medication as prescribed, including depots and clozapine.
- Obtain
requested routine bloods/investigations; have an understanding of normal and
altered values, reporting adverse results. The predominant focus for bloods is
Clozapine clinic.
- Provide
and promote health education; support and encourage the patient to meet their
own health and wellbeing needs.
- Implement
and contribute care plans and risk assessments within the limits of own
knowledge and competence.
- Assist
and/or deliver groups to promote patient lifestyle and independence.
- Complete
documentation accurately and contribute to reports of patients activity and
progress
- Participate
in discussions about patient care including MDTs and supervision.
- Maintain
and develop skills through training and supervision.
- Liaise
effectively and work collaboratively with colleagues both in the team and
outside of the team.
