We are looking to recruit an enthusiastic and committed Band 5 Staff Nurse to be based at Droitwich medical centre, which is situated close to junction 5/6 of M5,we also have another satellite hub in Tenbury located at the hospital , both offer free carparking . Our team covers 8 GP surgeries situated in the beautiful countryside of Worcestershire.

Main duties of the job

The role will require:

  • The ability to communicate effectively with others at all levels
  • Working flexibly to provide the service delivery need across the local area (the service operates from 7:30am 6:30pm, Monday to Sunday including bank holidays)
  • An enthusiastic, motivated and innovative thinker to become part of a primary care facing community
  • The ability to work autonomously and manage your own patient list whilst supervising others and working within a multi-disciplinary team
  • The ability to problem solve and manage time effectively
  • Experience of working in a fast paced and varied environment
  • The ability to travel to deliver care services

Work as an autonomous practitioner within the integrated team providing rapid and planned assessment and treatment interventions to patients in their own homes or other community settings to enable patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible. This will be achieved through a combination of hands-on clinical skills and knowledge to treat, educate and support patients on a designated caseload and by working with patients and their carers to assess, plan and implement appropriate packages of care.
