We are looking for an
enthusiastic, dedicated and motivated RMN who would like the opportunity to
work in a Development post within our Older Adult Community Mental Health Team,
and work with patients that have functional and organic mental health
needs.
The core hours are Mon-Fri (9pm -5pm)
and this specific post is for 22.5 hours per week over 5 days with one day
being a shorter day to enable online learning opportunities and adequate time
for admin.
You
will be part of an integrated team consisting of Community Mental Health
Nurses, Occupational Therapists, Consultant Psychiatrists, Junior Medics, a
Clinical Psychologist, Support Workers, a Trainee Advanced Clinical
Practitioner and Administration staff. You will also be supported by the Team
Manager who offers a robust supervision and appraisal process.

Main duties of the job
Work with patients to identify
therapeutic goals as part of the overall recovery-focused care plan, using
specialist mental health skills.
Effective and timely discharge
planning and coordination.
Ensure timely reviews of care
plans and assessments.
Undertake and
understand physical health observations and take appropriate action where
necessary.
Manage a
caseload of patients with complex mental health needs (taking on the role of
care coordinator where relevant).
Plan and
implement patient-centred individual and/or group interventions to achieve
therapeutic goals.
Monitor,
evaluate, and modify treatment in order to measure progress and ensure the
effectiveness of an intervention.
Apply a high
level of understanding of the effect of mental health needs and provide
training and advice on management of the condition, healthy lifestyles and
well-being.
Provide
education, consultation and advice to other care settings
Apply in depth
knowledge of any relevant health and social care Legislation and implications
on intervention.
Carry out
complex risk assessment.
Timely and
accurate reporting and escalation of incidents and safeguarding concerns.
Participate in
undertaking professional cover/duty.
To undertake
depot injections.
To work closely
with the MHA and MCA.
