Job summary

Herefordshire and Worcestershire Neighbourhood Mental Health Teams have an exciting opportunity for two full time (37.5 hours) candidates to work with Project Bright Path, the Mental Health Rough Sleeper Project. We are looking to recruit a candidate for Herefordshire and Worcestershire.

Working as part of a multi-disciplinary approach to supporting rough sleepers across the County. The project provides the opportunity to tackle the ongoing mental health needs of vulnerable rough sleepers who have fallen out of services, or who will not engage with traditional pathways of support. We are looking for an experienced Mental Health Nurse, Occupational Therapist or Social Worker, with a passion for making sure these people do not carry on living without access to appropriate support. A significant proportion of role will include working in the community directly with rough sleepers and advising front-line rough sleeper professionals on mental health issues.

Main duties of the job

Work closely with the Rough Sleeper Co-ordinators and Link workers to reach rough sleepers and those at risk of rough sleeping.

Provide intensive support to a small proportion of rough sleepers & those at-risk or rough sleeping alongside other professionals involved .

Work within existing MDT structures to agree support for individuals as part of a MDT process.

Provide mental health advice to front-line workers in RS and related organisations regarding risk formulation and strategy to support individuals.

Run reflective practice sessions for front-line workers in RS and related organisations to explore specific cases and facilitate learning

To contribute to the delivery of the Mental Health Rough Sleeper training programme

Build relationships with rough sleepers and those at-risk and identify unmet mental health need

Develop links with wider rough sleeping and homelessness strategic networks and service including Drug and Alcohol provision and Mental Health Response Vehicle Programme .
