Job summary
Are you passionate about supporting people with complex mental health needs?We now have an exciting opportunity for a highly motivated community Support Worker, to work with the Herefordshire Assertive Outreach Team (AOT). AOT support service users who have found it difficult to engage with community mental health teams. Serving people who experience psychosis and have serious complex and enduring mental health problems, who may have co-morbid substance use issues, are difficult to engage, and have found it hard to work with mainstream mental health services, often rejecting support while their needs remain very prominent.
We are a supportive team, motivated to achieve the best care possible and are keen to help colleagues to develop. As a Trust, we provide extensive training with opportunities to develop. The team provides a compassionate leadership approach and as part of this approach you will be expected to engage in regular supervision.This is a full-time, permanent post, 37.5 hours per week, over 5 days.
Main duties of the job
You will be passionate about making a difference to peoples lives and support the clinicians in supporting some of the most vulnerable people we are responsible for, who have complex ongoing needs.
Under the supervision of the clinicians, participate in the assessment, evaluation and monitoring of all care and support, provided by community placements.
Complete documentation accurately and contribute to reports of patients activity and progress
Participate in discussions about patient care including MDTs and supervision.
Maintain and develop skills through training and supervision.
Liaise effectively and work collaboratively with colleagues both in the team and outside of the team.
Anxiety management and graded exposure activities with support, as required and under the direction of the clinicians.
Helping clients to access and engage in community activities to help build a role and resilience.
