Job summary
The Richmond Response and Rehabilitation Team (RRRT) are looking for two experienced band 6 community nurses or paramedics looking to broaden their experience within the UCR services in Richmond.
RRRT are keen to support any personal or continued professional development of nurses seeking to develop their skills and clinical expertise within the community.
RRRT aims to reduce avoidable hospital admission, reduce hospital length of stay and support patients at home in line with service objectives. This role works within a multi-disciplinary, multi-agency team and in partnership with primary health care and social care professionals, providing expert case management, clinical assessment, diagnosis and individualised treatment plans, promoting self-management.
The post holder will be responding to the initial crisis, acute exacerbation or initial diagnosis to support the patient at home. Close liason with MDT may include but is not exhaustive of: GPs, virtual wards, acute hospitals, district nurses, community matrons and social workers,
Main duties of the job
- To be lead practitioners in undertaking Rapid Response cases within the Community
- Carry out a range of nursing interventions, working within the scope of professional practice.
- Provide expert management for patients, often with a range of complex long-term conditions, as part of the Richmond Response & Rehabilitation Service case coordination structure.
Working autonomously with advanced assessment decision-making and problem solving skills identifying and signposting patients to appropriate services
