Job summary

We are seeking an experienced autonomous practitioner who has the passion and skills for improving the care of people with Type 2 diabetes (and Type 1 diabetes in special circumstances). The role would suit someone who is highly motivated with a positive approach to high quality care delivery with excellent communication and leadership skills to work in partnership with our community and primary care colleagues.

Experience of working with virtual technology to support a flexible service in complex diabetes clinics and group settings would be welcome. They will act as a specialist resource working alongside and supporting community staff to meet the needs of people with diabetes in all community settings within the Borough including GP Practices/PCNs, Nursing Homes, housebound patients and patients with special needs etc. They will play a lead role in helping practices implement evidence-based practice and will contribute to the provision of a range of services to those with diabetes, participating in the management and delivery of treatment plans as necessary

The service is supported by Diabetes Consultant-led and GP with Extended Role (GPwER) diabetes clinics.

Diabetes Specialist Dietitians work closely with the Diabetes Specialist Nurses and also oversee the delivery of our structured education programmes for patients with Type 2 diabetes.

Main duties of the job

  • The post holder will work effectively as a member of the multidisciplinary team which includes patients, Specialist Diabetes Nurses, Diabetes Specialist Dieticians, Diabetes Consultant, GPwER in diabetes, primary care colleagues and community teams, to improve holistic health outcomes for patients with diabetes.
  • Provide specialist care, advice and support through direct booking primary care hubs (PCN) and jointly support the development of integrated working.
  • Provide joint practice-based clinic sessions to optimise care planning and prescribing decisions for complex patients in GP practices.
  • Triage and audit referrals to the service and provide relevant service and evidence-based feedback to referrers to provide timely care to patients.
