Job summary

We are looking for a senior nurse who is passionate about the care and services we provide for people who are coming towards the end of their life working within the Richmond Locality.

The Palliative and End of Life Care Lead hold the Trust's end of life care portfolio and will coordinate and lead the programme for end-of-life care within the Trust.

The post holder will function as a critical link with health and care partners to ensure end of life care is seamless for the patients we serve. The post holder will be critical in the maintenance and development of the Trust's strategy and local policies and will be a key contributor to the end of life care governance framework, ensuring the Trust strategy is in line with national guidance and will give assurance that end of life care is safe, effective, caring, responsive and well led within the Trust.

Main duties of the job

Support the development of key strategies aligned to end of life care; dementia, long term conditions; frailty in line with national guidelines and local requirements.

Develop and review the palliative and end of life care associated policies, guidance and EPR digital platform.

Work with key clinical staff to coordinate the development of a specific palliative and end of life care pathway for patients and carers across the Trust.

Support and develop a process of a seamless transfer of care between hospitals and community service to ensure continuity of care.

Develop palliative care with a focus on identification of deterioration and the management of advance and urgent care planning.

Working with senior clinical leads within the Trust and local health and care community, to ensure delivery and development of the local palliative and end of life care programme.

Ensuring best practice is incorporated into the end-of-life care plan.

Ensuring the maintenance of the governance and assurance framework for end-of-life care.

Developing and maintain robust KPIs within the Trust to give assurance regarding performance and quality of end-of-life care.

Coordinating the end-of-life care clinical audit programme to give assurance regarding the quality-of-care delivery and identify gaps in service for operational managers.

Working with senior nursing leads and palliative care providers to develop and establish a training and development programmes

This is an exciting role that presents opportunity to lead Palliative and End of Life Care in the community covering the Richmond locality

We are eager to welcome an inspirational Palliative / End of life Care Lead who will be working in the community and in partnership with acute partners, ICB networks and other health and social care professionals.

Working with senior nursing leads and palliative care providers to develop and establish a training and development programme for clinical and support staff which is in line with clinical professional standards and supports nurse revalidation and clinical supervision.

Using the end-of-life care measures (clinical audit, KPIs and patient and carer feedback) to inform end of life care commissioning.

Building on local partnerships within the health and care community to ensure there are robust links across all agencies to provide seamless care for patients

  • Educated with a Masters Degree
  • Specialist Palliative/End of Life Care post registration qualification

  • NMC recognised Teaching Qualification
