Job summary
We are currently looking for a 2 x Band 3 Therapy Assistants to work across the Richmond area. A shift system is operated to cover 8am - 8pm, 7 days a week.
You should be motivated to deliver high quality skilled care within the community setting to meet the needs of the housebound patient and in a clinic setting.
You will have good communication skills and be able to work autonomously but also work in an integrated way.
The Richmond Response and Rehabilitation Team will perform the following functions:
- Hospital discharge and early supported discharge.
- Crisis and Rapid Response.
- Community Rehabilitation support.
Access to a motor vehicle with appropriate driving license is essential. Please ensure that your application demonstrates that you meet the essential criteria as listed in the job specification.
Main duties of the job
Carry out client risk assessment as agreed with the occupational therapists and physiotherapists and provide feedback on clients' physical and occupational performance and skills deficits.
Carry out client treatment and ongoing rehabilitation as agreed with the occupational therapists and physiotherapists and as per service protocol and feedback on clients' physical and occupational performance.
Provide client instruction, direction and guidance during assessment and intervention and adjust own clinical interventions based on clients' needs during the intervention.
To evaluate effectiveness of the intervention in discussion with supervising therapist, and modify programme accordingly.Provide above client interventions in the patients home or in a community setting as appropriate.

About us
Hounslow and Richmond Community Healthcare NHS Trust (HRCH) provide a wide range of healthcare services for the population of London Borough of Richmond upon Thames and the London Borough of Hounslow.
The Clinical Services Directorates have a wide range of responsibilities to manage, including co-ordinating and delivering community primary care services on behalf of the Trust. A wide range of these healthcare services are delivered by staff in the community, who include medical staff, paediatric therapists, therapy assistants, health visitors, specialist nurses, nurses, nursery nurses, paediatric phlebotomists and healthcare support workers with the support of administrative staff working in localities with GPs, other health professionals, early years, education, and social care professionals.
The digitalisation of health services is underway with a requirement to become paper-light by 2023 through improving the use of digital tools and adopting mobile and agile working practices and increasing the efficiency and effectiveness of clinical delivery.
