Job summary

This post is funded by the NHSE additional roles' reimbursement scheme in the PCN DES GP Contract 2023/24. The post holder will be an integral part of the Hounslow Primary Care Networks (PCNs) system, which comprise of 47 member practices across five PCNs. They will work within their clinical competencies to: -

  1. To deliver dietetic care to primarily patients with Type 2 diabetes across an allocated Hounslow Primary Care Network.
  2. To lead the delivery and management of structured diabetes group education for patients with Type 2 diabetes and diabetes prevention across an allocated Hounslow Primary Care Network.
  1. To work as part of the Hounslow Community Diabetes Service Team to deliver complex Type 2 diabetes management support.

Hounslow & Richmond Community Healthcare NHS Trust will host the role. However, it is mostly GP practice based and the successful applicants will have the opportunity to embed the role of the PCN dietitian within the PCNs.

The Dietitian will proactively identify, assess, and treat primarily a diabetic caseload in primary care.

The Dietitian will fully integrate within existing PCN teams and become the source of expert Dietetic support and be supported by a named mentor within primary care.

The Dietitian will also work within the Hounslow Community Diabetes Service as part of the specialist diabetes dietitian team.

Main duties of the job

  • To deliver dietetic care to primarily patients with Type 2 diabetes across an allocated Hounslow Primary Care Network.
  • Work with the PCN to identify areas where patients need dietetic support and run specific PCN clinics / group sessions when required or signposting to relevant services.
  • Implement and deliver the PCN structured diabetes group education service through planning, setting objectives, and reviewing performance against targets.
  • Provide nutrition and lifestyle education to patients with Type 2 diabetes on a one-to-one clinic or within group settings in the Hounslow Community Diabetes Service. This will be one day a week only.
  • Regularly maintain and review plans, communicating progress to the PCNs and taking timely corrective action to ensure delivery of milestones.
  • To initiate appropriate Dietetic treatment and/or referral, and to signpost to other appropriate services.
  • To provide high quality, dietetic advice enabling patients to make informed choices about their own health.
  • To provide a source of expert knowledge and advice on nutrition to the PCN.
  • To be active and fully contribute to MDT discussions about medically, socially, and ethically challenging patients.
  • Promote proactive nutritional care to reduce attendances to GP practices and hospital.
  • To be aware of, identify and raise safeguarding issues observed during clinical practice.
