Community Nurse - Learning Disability (Band 6)
The post holder will work as an integral member of the Hounslow Adult Learning Disability Health Team (HALDHT), whilst having responsibility for a designated specialist caseload. The post holder is expected to positively represent the needs and rights of people with learning disabilities, tackling inequalities in health and improving access to identified services in the statutory and 3rd sector
Main duties of the job
As a specialist nurse, assess, plan, implement, monitor and evaluate the care/support needs and plans of individuals (undertaking relevant risk assessments)
Co-ordinates and undertakes visiting depot clinics, provides specialist advice in relation to the health and wider needs of adults with learning disabilities. Acts as a CPA coordinator where required
Forms a supportive relationship with learning disability service users, families and carers to facilitate care and support in all settings and collaborates with GP's, primary care teams, acute & mental health Trusts and other stake holders towards improving access to their services by people with learning disabilities
Provide clinical oversight (day to day and/or by clinical supervision) to junior nurses and student learners. Deputize for the Senior Community Nurse and Consultant Nurse and undertake designated duties when required

In 2021, the Hounslow Adult Learning Disability Health Team (HALDHT) received national recognition and was 'highly commended'by PrescQIPP, on the positive work the service has been been doing with STOMP and reducing the overuse/prescribing of medications in people with learning disabilities. In 2022, the service also reached the finals of the National Learning Disability and Autism Awards

- RNLD (Registered Nurse Learning Disability) Qualification & active on NMC Register
- Professional knowledge to degree level (e.g. Bachelor's degree) in a related health field (e.g. nursing, healthcare etc) or Diploma level training or equivalent in specialist area
- Evidence of continuing Personal and Professional Development

- Post qualifying experience (3 years) in health/ social care settings, possessing specialist knowledge of and applying clinical skills to clients with complex health/ mental health/ challenging needs

- Experience of contributing to service wide projects
- Experience of working in a multi-disciplinary team
- Experience of teaching training and / or supervision
