Paediatric Diabetes Nurse
Band 6 (£39,959 - £48,117 pro rata, per annum)
Part-Time (30 hours per week)
We are looking for an enthusiastic Paediatric Diabetes Nurse to join our MDT in caring for children and young people with diabetes on the Isle of Wight. We would welcome applicants with relevant paediatric and diabetes experience and the use of a car to facilitate community-based visits to help support patients to live their fullest life.
Main duties of the job
To assist in the co-ordination of care for children, families and young people who have diabetes.
To take a lead role in facilitating early discharge, prevention of attendance or admission to hospital.
To promote care as close to home as possible and provide skilled support to children/young people and their families within the community setting.
To act as a resource within the multi-disciplinary team to provide safe and appropriate management of the child's/young person's nursing needs.
To act as a vital link with acute services, schools, nurseries and GP's to provide safe and effective care to children/young people with diabetes.
To participate in research and audit with East of England Diabetes Service.
- To participate in on-call rota to support families
