Secondment Opportunity

An exciting secondment opportunity has arisen for an experienced enthusiastic and motivated Registered Nurse to join our diverse Critical Care Outreach Service. This Critical Care Outreach post is an excellent opportunity to advance your clinical knowledge, develop clinical examination and diagnostic skills while caring for acutely deteriorating patients within our Trust. The successful applicant will join a supportive and dynamic team, leading on delivering high standards of care to acutely unwell patients at our hospital. There is an opportunity to undertake our Critical Care outreach induction / training programme as well learning to work to the level expected of an Advanced Clinical Practitioner.

The Secondment is open to Registered Nurses and Allied Health Professionals with relevant critical care experience.

Job responsibilities

To provide highly specialist person centred care which always considers peoples safety, privacy and dignity using advanced clinical knowledge that is evidence based.

As part of a multi-professional team, the post holder will practice autonomously, whilst being accountable and self-directed in line with the relevant code of professional conduct.

To plan and manage complete episodes of care; undertake independent comprehensive assessment and management of service users to incorporate pharmacological considerations; making complex clinical decisions regarding service user management and clinical outcomes.

To provide expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision-making skills.

Participate in change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research and education. To promote and demonstrate best practice by integrating evidence into practice.

Contribute to the planning and development of integrated services.

The successful candidate will be expected to :

Collaboratively work as a fundamental member of the Multidisciplinary Team to providing safe and effective care in ward areas.

Recognise and respond to acutely ill and deteriorating patients ensuring timely interventions.

Respond to clinical emergencies as they arise throughout the hospital.

Ensure discharge patients from the Intensive Care Unit are reviewed in a timely manner, progressing well and assess risk of deterioration / readmission.

Support / deliver education to ward Nurses and AHPs
