To work autonomously as a clinical specialist nurse/AHP to provide advanced assessment, clinical diagnosis and treatment of patients within the acute frailty pathway.

This post will work primarily within the emergency care floor and in-reach into acute wards, liaising appropriately with acute therapy and nursing teams in order to assess needs, providing same day services, as well as supporting the effective use of alternatives to acute care including SDEC, virtual wards and community rapid response services.

  • Demonstrated ability to exert moderate physical effort for several short, and occasionally long periods during the working day.
