This is a truly exciting time to join our team as we work towards launching our newly developed £10m Community Diagnostic Centre (CDC). The new centre will have the ability to deliver joined up diagnostic care, and with increased capacity this will ensure our patients will have faster access to treatment, earlier diagnosis, and improved outcomes. As part of this national programme, our services need to ensure they are delivering "Quality" care in a coordinated and cohesive manner that ultimately leads to improved patient experience and outcomes.
We are looking for an individual to provide Clinical leadership in the strategic development of the Islands CDC, enabling it to reach its objectives in delivering new pathways of care that coordinate our services and support direct access and one-stop delivery. The post holder should have a robust awareness of the CDC programme and its aims in order to actively manage and provide clinical support to the services within it. The Island CDC shall work as "one" service across multiple specialties so we need a post-holder with a clear vision for how this can be clinically led and co-designed.

The post holder should have experience in service improvement, with well-developed interpersonal and communication skills as they will have the opportunity to work across all diagnostic services to deliver change initiatives. It is expected that the candidate is actively studying for their PHD and able to evidence their development against the 5 pillars of advanced clinical practice. The post-holder will serve as a conduit between Primary Care, Secondary Care, the ICB, and wider healthcare system in order to clinically engage our partners, and to ensure that any change is for the benefit of our population.
This is a fantastic opportunity to support the development of a new service, creating new pathways of care that provide prompt access and early diagnosis. Ultimately, making our Island NHS more resilient and accessible. We need a clinical leader to shape and drive this change.
To plan and manage complete episodes of care; undertake independent comprehensive assessment and management of service users to incorporate pharmacological considerations; making complex clinical decisions regarding service user management and clinical outcomes. To provide expert clinical advice, leadership, and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and advanced decision-making skills.
Lead system changes, to improve standards through supervision, evidence-based practice, clinical audit, research, and education. To develop and demonstrate best practice by integrating evidence into practice. Lead on the planning and development of integrated Community Diagnostic services at a service and strategic level.
To work within the five pillars of advanced practice. To act as a lead in system changes. To provide clinical leadership within the Islands Community Diagnostic Centre.
Lead in pathway development, making access for patients and primary care more streamlined. This includes implementation of direct access, one stop clinics, and greater coordination of our diagnostic services.
Championing high quality care for our Island community.
This role is enacted as an autonomous senior clinical professional working at the expert level as defined in the Local Integrated Advanced Clinical Practice Framework. Clinical duties will be undertaken without direct supervision but in collaboration with a designated Lead Clinical Supervisor. The role will also work within the fifth pillar of advanced practice Strategic function for consultant practitioner, taking forward system changes and leading a clinical team.
