Job summary

We are recruiting for a Band 3 Healthcare Assistant who wishes to join #TeamIOWNHS. You should share our passion and drive to ensure that our patients receive quality and compassionate care and work with us to ensure the Trust continues its journey towards "great people, great place".

Applicants are invited from individuals with a background in care who have the desire to deliver quality care to patients in their own home. You will be working alongside registered nurses and other members of the Integrated Community Team.

This is an exciting opportunity to join the Community Nursing Service as it goes through a period of change. There may be further opportunities of training and development following the Trust's Career Development Programme.

Job responsibilities

Training will be given in the use of our patient electronic record SystmOne. Training will also be given in the administration of insulin and application of compression therapy.

You will be working as part of a team and you must have good interpersonal skills and the ability to use your own initiative, work with minimum supervision to meet deadlines, and maintain a flexible approach to all aspects of the role.

To work as an integrated member of the Community Nursing Team, providing nursing care for patients in the community under the direction and supervision of the District Nurse Team Leader.

To observe complete confidentiality of all information, adhering to the legal requirements of the Data Protection Act and Trust information sharing policies.

Communicate with relatives/carers of patients in liaison with the District Nurse Team Leader, offering support and advice as necessary.

Report to the District Nurse Team Leader or deputy any changes or concerns in relation to patients visited.

Build and maintain, where possible, positive relationships with patients, carers and other colleagues.

Ensure that contemporaneous patient records are maintained, all paper entries are dated, timed and signed and electronic records are attributable to yourself.

Participate in team meetings including the wider Integrated Locality Meeting and other professional meetings/groups as requested.

Analysis and Judgement

Identify shortfalls in service provision for community nursing patients and bring them to the attention of the District Nurse Team Leader.

Planning and Organising

Take all possible steps to safeguard the welfare of staff, patients and carers, and be aware of the requirements of health and safety legislation. Report shortfalls to the District Nurse Team Leader. Take appropriate steps to promote own personal safety and that of other staff, patients and carers.

Prioritise and manage own work with minimal supervision, working to deadlines and time-scales despite frequent interruptions.

Ensure that your mobile device is updated on a regular basis and kept in good working order.

Ensure that you have adequate supplies with you to carry out your duties.

Patient and Client Care

Undertake patient care in a variety of community settings, as assessed, planned and agreed by the District Nurse Team Leader, maintaining agreed standards and quality of patient care.

Accept responsibility for the reasonable care of patients furniture and effects during the duration of the visit.

Within the scope of expanded roles and in consultation with the District Nurse Team Leader, take on new skills to enhance the care of patients within the community nursing team.

When a need for nursing aid or equipment is highlighted ensure where possible the required equipment is provided and, where this cannot be achieved, notify the District Nurse Team Leader. Always ensure patients, families or carers are instructed in safe use of equipment, who to contact if problems arise, and who to return it to when it is no longer required. Ensure instruction books are left with all equipment.

Encourage patients and their families to maximise their independence and well-being within your scope of practice.

Contribute and develop ideas to help improve the efficiency of the service.

Ensure the requirements of all Trust policies, protocols, clinical guidelines, care pathways and standing financial instructions are observed at all times.
