The learning disability liaison service forms part of the Community Adult Learning Disability Team and supports adults with learning disabilities to access services in our acute setting.

We are seeking an energetic and highly motivated individual who demonstrates a commitment to person-centred approaches and co-production.

Main duties of the job

The post holder will have key responsibility for providing a specialist learning disability liaison service which will include assessment, risk assessment, care planning, treatment planning and consultation for people with learning disabilities in the acute setting.

The Learning Disability Liaison Nurse will be part of the Community Learning Disability Team and work in alignment with them in terms of emergency and planned admission and discharge planning.

Job description

Job responsibilities

  1. Provide high quality assessment, care planning and nursing intervention for referrals made to the liaison service.
  2. Assist medical and nursing staff in the diagnosis and treatment of patients/clients who present to the service and develop safe and effective care practices within this field of speciality.
  3. Provide a prompt response to patient/client referral following jointly agreed procedures.
  4. Facilitate prompt referral to other health, social & voluntary services as appropriate.
  5. Use evidence-based practices & psychosocial interventions to facilitate, plan, implement and evaluate nursing care.
  6. Attend multi-disciplinary team meetings, best interest, MARM / safeguarding meetings, professionals meetings and clinical reviews as and when required.
  7. Promote a service that is safe, respects the views of the patient/client by actively promoting choice, engagement and involvement together in partnership with families and other agencies is fundamental to the delivery of the service.
  8. Develop partnership links and support the interface between the Trust and specialist learning disability services in order to help facilitate a smooth pathway through services. This will enable continuity for the patient and their carers/families and /or support workers when accessing both elective and emergency treatment. This will involve planning effective care whilst in hospital, then assisting with discharge/aftercare arrangements.
  9. Train and educate health professionals in the needs of people with a learning disability and in making reasonable adjustments to meet those needs.
  10. Supervise and manage junior staff working within the learning disability liaison service.
  11. Improve the discharge experience for patients, families, carers and long-term care providers (and help avoid delayed discharges) by liaising with community agencies and providing support where needs remains complex and risk of re-admission high.
  12. Compile and co-ordinate continued development in improving health care experiences for people with a learning disability and monitor and record reasonable adjustments.
