This post is vital in helping to enable people living with dementia and their families/carers to optimise independence through a range of assessment and therapeutic interventions. The aim being to enhance health and well-being, reduce risks and support engagement in meaningful activity within community settings.
This post requires the practitioner to work as part of a multi-disciplinary team within the Memory Service.
The post holder will manage a clinical caseload to deliver effective interventions within this speciality.
Interventions aim to have a significant impact on the wider economy in reducing hospital admissions and re-admissions, reducing residential placements, recommending appropriate support for domiciliary care packages as well as improving health outcomes.

To provide support and develop Memory Service in this specialist area and contribute to related policies.
You will demonstrate a high level of knowledge about dementia and co-morbidities and demonstrate skill and expertise relating to assessment of cognative ability, psychological and emotional wellbeing, risk assessment and management, and supporting the needs of family/carers.
To manage a defined caseload, using evidence based/patient centred principles to assess, plan, implement and evaluate interventions.
Will cascade knowledge through education and training within the Memory Service and the wider community.
To provide leadership for junior staff, through supervision and appraisal as delegated by the team lead.
To regularly supervise students on practice placement.
To contribute to the development of the Service by completing clinical audits, service improvements and undertake research.

The post holder will also be expected to work as an autonomous practitioner, as well as taking all opportunities to become actively involved in training and teaching programmes, both within the Trust and the wider community.
