An exciting opportunity has arisen for a committed, enthusiastic and experienced individual to join our forward-thinking, National award-winning Community Division in the South Locality of the Island, in the senior role of a Community Matron.

The Isle of Wight NHS Trust is an advancing Integrated Trust providing Acute, Mental Health, Ambulance and Community Services.

You would be joining our Community Division at an exciting time as we progress on our journey to outstanding, having achieved a CQC rating of good overall in 2019 including Outstanding for Care in 2021. Our transformational achievements include the extensive use of technology enabled care and the development of integrated multi-disciplinary and multi-agency teams, in conjunction with a wide range of partners across the Island and mainland.

Main duties of the job

This opportunity will be central to the continued drive for Quality and Improvement with a focus on person centered care. The post holder as well as being a senior clinician in the community nursing service, will take the lead on the management of Long-Term Conditions predominantly in one of the Island three Localities.

This senior leadership role also involves supporting the Integrated Team Leader for Nursing and Therapies in the locality as well as the ongoing development of other colleagues.

Job description

Job responsibilities

To collaborate with other healthcare professionals to identify within locally agreed criteria the high-risk patients with long-term conditions suitable for intervention.

  • To provide evidence-based care plans focused on sound clinical decision making, including knowledge of the unique presentation of illness and response to the treatment of those with complex long-term conditions and ensure a robust process is in place to review these care plans.
  • To act as Case Manager for patients with continuing care needs including the management of personal healthcare budgets where identified.
  • To work as part of the integrated locality team to ensure robust care plans are in place and reviewed on a regular basis.
  • To attend case review meetings and promote multidisciplinary working.
  • To promote community care delivered at home, thus preventing unnecessary admissions to hospital and reduction in length of hospital inpatient stays.
  • Monitor indicators of long-term conditions, anticipating possible decline and proactively managing this to enhance well-being and maintain independence.
  • To actively support the use of Telehealth in order to promote self-management.
  • Plan interventions on a basis of needs, preference, choices and expressed wishes, facilitating discussion with the family and significant others.
  • To work as part of the In-Reach Service, developing good communication with the Acute sector to support early discharge and patient flow.
