Job summary

The Memory Service is offering an exciting opportunity for one year to develop and progress as a health care assistant. This is a fast paced out patient department for people with cognitive decline offering initial assessment, diagnosis, treatment and educational interventions to enable and empower a person to live the best life they can with a diagnosis of dementia.

Main duties of the job

You will need the skill to be able to engage with our service users, have excellent written and verbal communication skills and be an active listener.

You will work closely, supporting the qualified team of nurses, occupational therapist, psychologist and medics, delivering interventions and supporting written care plans.

You will need to have the ability to use your own initiative and to work in challenging and changing situations and be willing to learn and engage with new and exciting developments

You will need to be confident using IT skills and have a working knowledge of the RIO system.

You need to be working within the Trust values at all times

A full clean driving license is required for this role

Job description

Job responsibilities

  • To perform duties under the direction and indirect supervision of a named qualified professional
  • To recognise when to seek support or escalate a clients needs or risks to qualified team members
  • To undertake ECG, Pulse, Blood Pressure, Weight with patient either in hub or preferred community setting
  • To provide a follow up service either face to face, virtually or by phone and record accurately in electronic care records
  • To maintain positive relationships within the multidisciplinary team and all others involved in the care of the service users
  • To arrange/attend discussions regarding service user care as and when required and provide input into the evaluation of the collaborative care plan
  • To build therapeutic relationships with the service users/supporters and provide reassurance and information regarding the care pathway
  • To facilitate/co facilitate psychosocial and educational interventions with service users and their supporters
  • To encourage and enable service users to be fully involved and engaged with their agreed care plan and to access appropriate services
  • To promote equality for all individuals by promoting and supporting individuals rights and choice
