Job summary
The Primacy and Community Mental Health Service is delivered in a mix of on site and at home working (virtual appointment). The mental health practitioner role is expected to see 26 appointments each week with 5 new patients each week. The appointments are 45 mins long. You will provide treatment to patients which is planned is no more than 16 sessions in length. Patients are to be seen either weekly or Bi weekly, with future booked appointments booked well in advance.The mental health practitioner will need to conduct robust and comprehensive Biopsychosocial assessment of need for the individuals who present with a wide range of needs. This will then inform the treatment plan.The community mental health nurse/social worker/Occupational Therapist will have knowledge of working with individuals who experience suicide,self-harm, acute mental illness presentations and Dementia / confusion /delirium.
Main duties of the job
Able to work independently in a number of environments as an autonomous practitioner.Provide a comprehensive specialist mental health assessment and identification of a referred patients mental health needs, formulating and implementing nursing care plans and goals which will reflect the needs of the patientsRegularly review caseload management with their clinical supervisorCarry out procedures in accordance with best practice and within own skill set or under supervision of others.Maintain accurate records about the care and treatment provided.Complete with patients outcome measures and encourage the completion of patient experience measures.Review and evaluate professional standards in conjunction with the quality TeamEnsure that the developing requirements of the Mental Health Nursing/ social work/ Occupational therapy profession are understood by the wider team
