Job summary

An exciting opportunity has arisen to work within our Older Adult Community Mental Health Teams based at Edmund Road. The Older Adult Community Mental Health Team supports people aged 65 and over who are experiencing mental health difficulties.

We support people who are experiencing problems such as psychosis, mood disturbance and memory problems, who have related behavioural or psychological symptoms.

The team covers the entire city. We provide support in local communities, including home visits or visits to care homes.

In May 2021 we were delighted to find out that we had been successfully accredited withtheRoyal College of Psychiatry.

The accreditation programme works with staff to assure and improve the quality of community mental health services for people with mental health problems and their carers.

Older Adult Community Services are currently engaged in a process of transformation.

You should be able to demonstrate your passion of working with this client group and have the ability to assess, co-ordinate treatment and support clients and carers. You should possess effective communication skills and have the ability to work under your own initiative and as part of the wider Multidisciplinary team.

A professional qualification in Mental Health Nursing is essential.

Main duties of the job

  • To work across a variety of community settings as part of a multi-disciplinary team.
  • Providing a nursing assessment and identification of clients' mental health needs .
  • Formulating, implementing and evaluating nursing care plans that will reflect the needs of the clients/carers over a 24-hour period.
  • Manage own caseload having responsibility for the nursing care of clients until point of discharge.
  • Administering depot injections to clients and monitoring the effects of prescribed medication.
  • To act as co-ordinator /key worker
  • Delivering a range of therapeutic interventions with clients and carers in accordance with evidence based practice and Trust Policies.
  • Reacting to and managing a range of complex mental and physical health crises.
  • Offering support and advice to families and carers.
  • Managing and maintaining a caseload with appropriate levels of

    • To provide direct supervision to other staff and contribute and assist with PDR system.
