Job summary

To work as part of the Sheffield Eating Disorders Service Multi Disciplinary Team based at St George's Community Health Centre. The post holder will work as part of the SEDS team implementing and planning care for our service users with eating disorders, in collaboration with the MDT.

The post holder will work with patients who are attending St George's, in their own homes and within the acute hospitals.

The post holder will work in partnership with service users and other agencies to ensure effective liaison and communication.

The successful applicant will maintain professional responsibility whilst exercising autonomous professional and clinical judgement within the overall framework of SHSC, and the relevant professional registration bodies (NMC).

Band 5 development post will be considered

Main duties of the job

  1. Undertaking holistic assessments including risk assessment as part of the Scheduled Care Pathway. Escalating perceived assessed risk to the MDT and developing treatment packages based on a collaborative formulation of the clients' needs.
  2. Ensure that all clients have a clearly defined and agreed collaborative care plan, designed to meet their needs and to provide the agreed intervention.
  3. Monitoring the care and treatment and wellbeing of clients with eating disorders and liaising where appropriate with primary care and other services.
  4. Collecting relevant data e.g. PROMS to evaluate and review treatment at appropriate intervals using both client's self-report and service outcome measures. Ensure all clients receive appropriate follow up.
  5. Case management and liaison with services/clients and any other agencies that may be identified.
  6. Providing progress reports and maintaining effective communication and liaison with referrers.
  7. Maintaining appropriate health records as per the service and Trust policy.
  8. Managing a caseload in line with relevant policies.
  9. Working with clients who have eating disorders and who require ongoing support to manage their needs.
  10. Attend and participate in multidisciplinary team meetings and training sessions as required and directed.
  11. To participate in clinical and Care Planning Meeting outside of the Eating Disorders Service to provide specialist advice and facilitate cross-referrals.

Job responsibilities

This position is for a Registered Mental Health Nurse (RMN) with additional training, experience or a special interest in eating disorders. The role requires an individual who is committed to team working and has the flexibility to work with clients who may be resistant or ambivalent about change on a one-to-one basis. This role requires a dynamic person who is able to work autonomously, whilst retaining responsibility for managing/planning and caring for a group of acute and chronically unwell patients. This role will include a requirement for training which will be provided to support the post holder to be competent in carrying out required physical health observations. The post holder will work closely with an experienced multi-disciplinary team who will provide ongoing training, direct supervision and support. The post holder will provide advice and consultation to primary care, mental health teams, acute hospitals and other relevant agencies.

  1. To provide a specialist eating disorders clinical service, which includes managing an agreed caseload inclusive of managing and supporting mental and physical health risk.
  2. Clinical work includes developing eating disorder treatment care plans alongside the MDT.
  3. Working alongside other team members, you will be supported to participate in the triage of incoming referrals to the service.
