Job summary
We have an exciting new opportunity within the established and highly regarded Neurological Case Management Service (NCMS). This post will be band 7, 22.5 hours per week.
NCMS work closely with the Neuro enablement (NES) and Community Brain Injury Team (SCBIRT) to improve quality of life for our patients in the most complex circumstances.
This case manager role will co-work closely with SCBIRT and have a particular focus and responsibility for patients with brain injuries (traumatic and acquired). This is an exciting service development opportunity to help patients with a range of brain injuries to optimise engagement with SCBIRT rehabilitation and wider agencies. You will need to contribute to the care and support of service users with non-brain injury long-term neurological conditions as required.
We are looking for a skilled clinician AHP Professionals or Registered Nurse (RGN or RMN) with an expressed interest and commitment to service development. You will be keen to contribute to change and to shape our services for the future.

You will already have experience of coordinating a caseload and have an awareness of the issues that people with brain injuries face. You will receive peer support from the existing band 7 Neuro Case Managers and regular consultation/ supervision with a Clinical Neuropsychologist.
Main duties of the job
To manage a defined caseload of community based clients with highlycomplex needs resulting from congenital or acquired (progressive ortraumatic) neurological disorders, using highly specialist skills andevidence based / patient centred principles to formulate care plans andoversee their implementation, monitor and review the individual'songoing needs and the appropriateness of the service(s) in order toimprove quality and access across health and social care, promotinguser choice and independence.
To participate in the planning, development and evaluation of serviceswithin a designated area/team, holding responsibility for definedprojects in line with service and organisational Development.

To develop and provide highly specialist clinical expertise in thespecialism of Case Management of Long Term NeurologicalConditions (LTNC), participate in the dissemination of evidence basedpractice across the service and to be a specialist resource and anintegral member of the inter-disciplinary and multi-agency teamsacross health and social care networks and non-statutory providers
To understand and apply current and relevant local and nationalguidelines and legislation relating to health and social caredevelopments
