Job summary

To value and be responsive to individual's rights, needs, beliefs and opinions for the Older Adult client group.

Be responsible under direct, and indirect supervision for providing individual care to Older Adults in a variety of settings, whilst maintaining and promoting respect, dignity, and choice at all times.

To maintain and promote a person centred care approach in all aspects of service delivery to the Older Adult using our service.

Main duties of the job

To implement identified plans of care for assigned clients under the direction/supervision of a named qualified professional.

To respond in a timely manner to the changing needs of the client group, within the scope of knowledge and skills.

Maintain accurate record keeping. To report actions/observations in a timely manner to a named qualified professional to ensure a thorough evaluation of care takes place.

To identify and report any untoward incidents at earliest opportunity to a qualified professional.

Job responsibilities

Duties and responsibilities are to be performed under the direct/indirect supervision of a named qualified professional.

To maintain positive relationships within the multi-disciplinary team, and anyone else involved in the care of the client.

To maintain the dignity of the client at all times.

To escort clients to other care/social environments at times, following a risk assessment, and in collaboration with Occupational health if required.

To assist in helping clients to reach agreed care plan goals.

To assist and carry out referrals to other services under the guidance of a qualified professional.

To assist qualified professionals in the organisation and delivery of group therapeutic activities.

To undertake physical health screening as advised by a qualified professional.

To support with students and other visitors that spend time with the team.

To organise time effectively, working flexibly to service requirements within contracted hours.

To be proactive with clinical supervision and personal development reviews.

To ensure you are meeting all training requirements of your role.

To be actively participating in multi- disciplinary team meetings, and any other meetings require attendance

To work within trust policies & procedures.

To be aware of the risks associated with being a lone worker within the community and making sure you are adhering to the lone worker policy.
