Job summary

We are seeking a dynamic, enthusiastic and motivated Mental Health Practitioner to work within the North Recovery Team, We are looking for experienced clinicians who can lead and support specific areas of service development. You will need a relevant qualification in mental health nursing or social work with experience of working in a multi-disciplinary mental health setting.

Main duties of the job

Skills in assessing, planning, evaluating and managing care within the Care Programme Approach are essential as is a working knowledge of social care management. You will need to possess excellent communication skills and be able to work independently and as part of the wider team. You should have a positive approach to meeting the needs of our service users and be able to demonstrate continued professional development.The successful individuals will also be expected to work as part of the duty rota which operates within the general working hours of 09:00 to 17:00 Monday to Friday.

Job responsibilities

The post holder will work within a multi-disciplinary service and core responsibilities will include:

  • Acting as a Care Co-ordinator/Lead Professional
  • Managing a large Waiting List
  • Carrying out short interventions
  • Reviewing care packages
  • Comprehensive assessment of new referrals
  • Chairing MDT's
  • Contributing to a duty system on a rota basis
  • Delivering supervision to more junior members of staff
  • Contribute to Multidisciplinary service development

Clinical

  • Triage and determine urgency of new referrals to the service
  • Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service.
  • To assess, evaluate and manage clinical risk
  • To formulate a plan of care based on identified need
  • Deliver evidence based interventions as appropriate
  • Review effectiveness of care plan at regular intervals
  • To work within safeguarding processes, including where required to act as safeguarding investigator / manager.
  • To work collaboratively with service users, carers and partner agencies
  • To maintain computer held care records in an accurate and timely fashion
  • Undertake all work in a culturally sensitive manner
  • Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.
