The South Recovery Service alongside other Services are currently undergoing service review which the team are actively encouraged to contribute to. The service has recently reintroduced the Assertive Outreach Team alongside Structured Clinical Management in the Recovery Service and other opportunities for development and learning, we feel this is an exciting time to partake in the develop of care pathways.

The successful candidate will be supported with their professional and leadership development through dedicated professional supervision with highly experienced and motivated senior practitioners, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions and candidates are encouraged to bring their own interests to the team to share and develop their specific interests in mental health.

Main duties of the job

Assessing, planning, evaluating and managing care within the Care Programme Approach are essential as is a working knowledge of social care management. You will need to possess excellent communication skills and be able to work independently and as part of the wider team. You should have a positive approach to meeting the needs of our service users and be able to demonstrate continued professional development.The successful individuals will also be expected to work as part of the duty rota which operates within the general working hours of 09:00 to 17:00 Monday to Friday.

The post holder will work within a multi-disciplinary service and core responsibilities will include:

  • Acting as a Care Co-ordinator/Lead Professional
  • Working within Care Programme Approach
  • Comprehensive assessment of new referrals
  • Carrying a varied caseload defined by need
  • Contributing to a duty system on a rota basis
  • Delivering supervision to more junior members of staff
  • Contribute to Multidisciplinary service development

Clinical

  • Triage and determine urgency of new referrals to the service
  • Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service.
  • To assess, evaluate and manage clinical risk
  • To formulate a plan of care based on identified need
  • Deliver evidence based interventions as appropriate
  • Review effectiveness of care plan at regular intervals
  • To work within safeguarding processes, including where required to act as safeguarding investigator / manager.
  • To work collaboratively with service users, carers and partner agencies
  • To maintain computer held care records in an accurate and timely fashion
  • Undertake all work in a culturally sensitive manner
  • Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.
