The successful applicant will work alongside a team of neuro case managers to support our clients. You will also work closely with our other multidisciplinary neurological teams.

Main duties of the job
To manage a defined caseload of community based clients with highly complex needs resulting from congenital or acquired (progressive or traumatic) neurological disorders, using highly specialist skills and evidence based / patient centred principles to formulate care plans and oversee their implementation, monitor and review the individual's ongoing needs and the appropriateness of the service(s) in order to improve quality and access across health and social care, promoting user choice and independence.
To participate in the planning, development and evaluation of services within a designated area/team, holding responsibility for defined projects in line with service and organisational Development.
To develop and provide highly specialist clinical expertise in the specialism of Case Management of Long Term Neurological Conditions (LTNC), participate in the dissemination of evidence based practice across the service and to be a specialist resource and an integral member of the inter-disciplinary and multi-agency teams across health and social care networks and non-statutory providers
To understand and apply current and relevant local and national guidelines and legislation relating to health and social care developments
