Job summary
Birch Avenue Nursing Home provides 40 nursing home beds for people with complex dementia. The Registered Provider and owner is South Yorkshire Housing Association, and we work in partnership with Sheffield Health and Social Care Foundation Trust. The focus of the home is improvement and empowerment for the service user.
We are looking for a motivated caring person to join the team to support and provide assistance to our residents to ensure that they have quality and meaningful life here at Birch Avenue.
Previous experience working with those diagnosed with dementia is a desirable part to play, however, not essential as successful candidates will be given induction, training, and opportunities for personal and professional development.
As part of the team, you would be expected to be able to work shifts over a 24-hour basis including weekends and Bank Holidays, so a flexible approach is key.
Basic computer skills, verbal and written communication are essential to navigate this frontline care role.
Main duties of the job
To work flexibly to meet service requirements within contracted hours which will involve acting as duty manager, and also working shifts on clinical areas when the need arises. This will involve working some evenings and weekends and other variable hours should the need be identified.
To manage and lead junior staff in the delivery of high quality care to residents. To ensure the appropriate delegation of key tasks to junior staff.
To work to Professional Codes of Conduct, Care Quality Commission Outcomes and SHSC Policies and Procedures in conjunction to close working with relevant Housing Association and to ensure junior staff also comply with these standards.
Be personally responsible for maintaining professional registration with the NMC. Failure to produce evidence of periodic registration on time may lead to regrading/dismissal

The Deputy Manager / Clinical Nurse Educator will be responsible for the clinical leadership and management for one or more of the cottages. To supervise and assist the Nursing staff in providing continuing responsibility for the assessment, planning, implementation and evaluation of care programmes and the setting of standards for nursing practice.
Assist the Registered Manager and fellow members of the senior management team in ensuring the smooth running of the Home, including the maintenance of a clean, safe environment, and the provision of good quality care, activity and nutrition.
As duty manager to respond to any problems in these areas, in the absence of the Home Manager. Periods of absence such as annual leave, weekends, sick leave that will be shared amongst the Clinical Management Team.
To assess people on the waiting list to ensure the environment can meet their needs, and to ensure a smooth transition of care as they take up their tenancy by drawing up an initial care plan, communicating this to staff and supporting residents.
Through supervision of Band 5 nurses, to ensure that a detailed nursing assessment is carried out for new residents, and that they have a comprehensive, clear and up-to-date plan of care designed to meet the individuals identified care needs, and that these plans are regularly reviewed.
Quality assurance trough the auditing of care plans and drug charts, identifying best practice and identifying areas of improvement back to the senior clinical management team
To ensure the appropriate involvement of residents, their relatives and friends in their care and to ensure that clear, effective communication occurs with the person identified as the main carer, or appropriate contact, for the resident.
To maintain and improve communication systems within Birch Avenue, and with community professionals who come into the Home.
To provide clinical advice and expertise to colleagues in Birch Avenue, taking a lead role in particular areas, working with the Manager and Senior Management Team.
To promote opportunities for self, colleagues and junior staff to access available education or training.
To share identified skills with colleagues, learners and junior staff, and to lead formal and informal teaching as appropriate.
To support the mentoring of students, and to act as mentor.
