Job summary

We are looking for enthusiastic individuals who wish to join a developing front-line service. This is an opportunity to influence and improve pathways for adults who are experiencing mental health problems. We are a team of experienced, supportive and motivated staff.

This post is suitable for those with direct experience of providing practical support to clients with mental health problems. It would be an added bonus to have particular knowledge of community-bases services and groups within Sheffield.

It is an interesting time to be working with this team as we are leading the way in testing new ways of providing mental health interventions in primary care for adults with severe mental illness and complex needs. The Single Point of Access and Emotional Wellbeing Service is being developed as part of the Primary Care transformation project.

Main duties of the job

Main duties

  • Work closely alongside team members and peers, to assist in delivering evidence based client care.
  • Work with colleagues, providing practical support to help clients achieve their identified goals.
  • Implement collaborative programmes of care with clients and carers under the supervision of a named professional.
  • Use the Trust electronic systems to provide relevant information and support to clients.
  • Observe confidentiality regarding clients at all times.
  • Document clients' presentation, progress and any significant events in clinical notes and ensure that these entries are counter-signed by a registered clinician.
  • Contribute to ongoing risk assessments by identifying signs of recovery and relapse, monitoring clients' progress, level of functioning and mental state and communicating outcomes and conclusions with staff involved in their care.
  • Take a positive approach to responding to clients in acute distress adopting principles of diffusion and de-escalation

  • To contribute to the development of clinical audit team standards, Trust policies and clinical guidelines, ensuring that these are adhered to at all times.
