The South Recovery Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of the Sheffield, this includes working towards a joint approach with Primary Care and the Voluntary Sector, reviewing the CMHT Service Criteria and working in discreet small teams into local communities.

The successful candidate will be supported with their professional and leadership development through dedicated supervision with highly experienced and motivated team managers, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions and candidates are encouraged to bring their own interests to the team to share and develop their specific interests in mental health.
Main duties of the job
You will need to possess excellent communication skills and be able to work independently and as part of the wider team in order to assess, plan and evaluate care. You should have a positive approach to meeting the needs of our service users and be motivated, empowered and enthusiastic to both learn and share.
You will also be expected to work as part of the duty system which operates within the general working hours of 09:00 to 17:00 Monday to Friday on a rotational basis. This 'mini team' is supported by other clinical professional and management.

The post holder will work within a multi-disciplinary service and core responsibilities will include:
- Acting as a Named Worker
- Comprehensive assessment of new referrals
- Carrying a varied caseload defined by need
- Contributing to a duty system on a rota basis
- Delivering supervision to more junior members of staff
- Contribute to Multidisciplinary service development
Clinical
- Triage and determine urgency of new referrals to the service
- Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service.
- To assess, evaluate and manage clinical risk
- To formulate a plan of care based on identified need
- Deliver evidence based interventions as appropriate
- Review effectiveness of care plan at regular intervals
- To work within safeguarding processes, including where required to act as safeguarding investigator / manager.
- To work collaboratively with service users, carers and partner agencies
- To maintain computer held care records in an accurate and timely fashion
- Undertake all work in a culturally sensitive manner
- Act in accordance with Care Management System, Acute and Schedule Care Pathways as appropriate.
