Job summary
A vacancy has arisen in the North Recovery Caseload Management Service which provides a service for a cohort of service users with ongoing health and social care needs. The post holder will have continuing responsibility for the provision of skilled intervention of people with assessed and diagnosed as having significant mental health problems in the community. The team is a friendly and dynamic one and provides a range of individual and group bases interventions including medicine clinic and social drop in. This is an exciting time to join the team as the service is currently planning for transformation and you will be actively encouraged to participate in this.
Main duties of the job
The post holder will work within a multi-disciplinary service under the supervision of experienced staff, and core responsibilities will include:
- Acting as a Care Co-ordinator/Lead Professional.
- Work within Care Programme Approach.
- Carrying a varied caseload defined by need.
- Contribute to a duty system on a rota basis.
- Participate in medicine clinic and social drop in

- Contribute to carrying out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action.
- To assess, evaluate and manage clinical risk.
- Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.
- To formulate a plan of care with service-users and carers based on identified need.
- Function as a member of a multi-disciplinary team.
- Deliver evidence-based interventions as appropriate.
- Review effectiveness of the care provided and where appropriate initiate action.
- Advises and supports the person and carers to promote health and well-being and to prevent illness.
