Job summary

This is an exciting opportunity for the right candidate to shape service & positively engage with and change the lives of service users who have a severe and enduring mental illness and who have disengaged with mental health services.

The Assertive Outreach Team will offer a city-wide service working Monday to Friday between the hours of 9am & 5pm.

You will need to have the enthusiasm for, and experience of, working with service users with complex needs & who experience difficulty in engaging with mainstream services.

You will be responsible for setting supervision and support systems to junior AOT staff in their delivery of interventions, and to deliver excellent evidence-based care to patients within the service

The temporary location for this role will be Fulwood House, Old Fulwood Rd, Sheffield S10 3TH.

Main duties of the job

  • Maintain NMC registration / re-validation and adhere to professional guidelines and code of conduct.
  • Work within the framework of relevant policy and legislation both local and national.
  • Will act as a practitioner with access to supervision of the Band 6 nurse within the AOT.
  • Will manage a defined caseload under supervision from the clinical lead within the AOT Undertaking risk assessment, intervention and appropriate escalation regards to presenting risk to senior team members and/or external agencies.
  • Will be responsible for the planning implementation and evaluation of nursing care.
  • Working as active member of the team and assisting in the assessment of patients referred to the AOT service.
  • Support senior colleagues in the effective assessment and the delivery of interventions to individuals within their assigned caseload.
