Job summary
The Early Intervention team provides support to clients from the ages of 16-65. We work with people experiencing their first episode of psychosis. We support clients and their families or carers for a period of up to 3 years and deliver interventions according to NHS England guidelines.
The post holder will work within our multi disciplinary team and core responsibilities will include:
- Acting as a Care Co-ordinator/Lead Professional
- Working within Care Programme Approach
- Carrying a varied caseload defined by need
- Contributing to a duty system on a rota basis
- Delivering supervision to more junior members of staff
- Contribute to service development
Main duties of the job
- Carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree the most appropriate course of action
- To assess, evaluate and manage clinical risk
- To formulate a plan of care based on identified need
- Deliver evidence based interventions as appropriate
- Complete physical health reviews
- Review effectiveness of care plan at regular intervals
- To work collaboratively with service users, carers and partner agencies
- To maintain computer held care records in an accurate and timely fashion
- Undertake all work in a culturally sensitive manner
- Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.
- Develop effective working relationships with key stakeholders
