Job summary

An exciting opportunity has arisen for a suitably experienced and highly motivated registered practitioner to lead the development and implementation of the clinical risk management and relevant components of the patient safety workplan on behalf of the Trust. You will also provide Sheffield Health and Social Care staff with expert support, information, training and guidance.

The successful applicant will have an in-depth knowledge and significant experience in clinical risk, suicide prevention and patient safety.

The post holder will report to the Head of Nursing for Rehabilitation and Specialist Services and work closely with members of the quality directorate, specifically the Head of Clinical Quality Standards and the Patient Safety Specialist, clinical services and corporate teams.

Main duties of the job

Lead on policy and procedural development including clinical risk management and suicide prevention.

Actively work with SHSC teams and partner agencies at local, regional, and national level to ensure the Trusts approach to clinical risk management, patient safety and suicide prevention is understood and included within related activity.

To take a lead in the development of a learning environment that will support change.

Undertake training needs analysis relating to clinical risk and suicide awareness and prevention for patient-facing staff. Develop training plans and packages rooted in evidence based / best practice with strong service user involvement in response to identified needs.

Use data triangulated from a variety of sources including local incidents and national evidence to inform the strategical direction of the Trust.

Professional and Clinical Leadership

Be an active member of the Suicide Reference Group and the Suicide Prevention Partnership both citywide and ICS.

Develop and maintain strong relationship with the Patient Safety Specialist to ensure a cohesive patient safety work plan aligned to the NSHEI Patient Safety Specialist.

Be a visible clinical presence to support advice and demonstrate high standards of patient safety and clinical risk management processes across the Trust. Provide expert knowledge and consultancy to other disciplines and professional groups.

Develop and monitor standards and audit to align with the required level of clinical practice/record keeping which will form part of the policy and governance of clinical risk, supporting with the identification of actions required to deliver an improved position.

Develop systems and processes within the Trust which aids the organisation to determine the level of assurance regarding the quality of care provided by SHSC services.

Utilise appropriate audit, research knowledge and clinical practice skills to lead, develop and inform education and training on patient safety.

Ensure clinical risk and suicide prevention training is delivered at an appropriate level commensurate with role and responsibility.

Develop and deliver suicide assessment and management training for clinical colleagues which ensures the inclusion of safety plans for all patients as a routine outcome from an assessment, which is shared with the service user and their family where appropriate.

Develop local training for clinical areas aligned to speciality with the support of the clinical services where specific patient safety issues are identified.

Support service leads to ensure that all in-patient wards have a robust system in place to manage environmental and clinical risks, specifically in relation to Ligature Assessments Points (LAP).

Provide clinical/professional and safeguarding supervision to staff as required.

Act as a human rights champion, receiving relevant training and role modelling use of a human rights framework in clinical practice.

In conjunction with the Risk Management and Health and Safety Team team, ensure that all incidents related to clinical and environmental risks are robustly investigated and reviewed, supporting with the identification of actions required to deliver an improved position.

May be requested by exception to lead in the investigation of highly complex or contentious serious incident investigations or complaints.

Service Development

Support the monitoring of national clinical safety indicators and development of local Trust indicators.

Keep oversight of the learning arising from incidents and incorporate within training in a timely manner.

Supporting the implementation of learning from national best practice, research and what has gone well as appropriate across the Trust.

Maintain up to date clinical knowledge and skills, demonstrating knowledge of current legislation and apply as relevant to practice.

Undertake benchmarking and advise the Trust on any required changes to clinical practice following the release of new strategies, initiatives and innovative approaches to patient safety, clinical risk management and suicide prevention.

Establish, lead and chair relevant working groups within the Trust to support the progression of Trust strategy and associated workplans.

Service User and Carer Experience

Training packages will be co-produced and co delivered with service users and experts by experience; they will have a strong influence over the development of the training and will be involved in co-delivery of the training at local level.

Be a strong advocate for service users and their families and carers in promoting knowledge, awareness, and confidence in staff to safely manage risk.

Professional Development

Demonstrate on-going personal development through participation in internal and external development opportunities, recording learning outcomes in a portfolio, to meet professional registration requirements.

Be a subject matter expert in the field of clinical risk and patient safety and develop an excellent knowledge of the local and national frameworks and associated tools used for improving and understanding patient safety.

Participate in management supervision with the Head of Nursing for Rehabilitation and Specialist Services. The post holder will have direct line management responsibilities for allocated staff and is expected to be a role model for clinical quality and to support staff in Clinical and Corporate Directorates to understand their duties with respect to Well Led organisations.

The post holder may offer mentoring and professional supervision to colleagues in the Trust.
