Job summary

Community Mental Health Services are undergoing widespread, innovative transformation and investment over the next 3 years. Camden & Islington NHS Foundation Trust are at the forefront of this transformation, developing a more holistic, recovery oriented, biopsychosocial approach to supporting people with Mental illnesses.

In partnership with the Primary Care, other NHS providers, Local Authority and the Voluntary Sector providers. The aim is to build support systems that reach the whole mental health population with the focus on prevention and sustained recovery delivered by a multi-agency collaboration. The core team will be based on the footprint of the primary care network, bringing together NHS, voluntary sector, peer support and community organisations. These relations will be central to the core team.

The role includes working with people who are more complex and beyond the expertise of GPs but do not meet thresholds for or find it difficult to access specialist, secondary care mental health services.

Population health nurse is a senior specialist nurse role and will be expected to work across the statutory and the non-statutory boundaries in Camden and is central to the success of the development of Core team. This is a high profile, outward facing post focusing on supporting colleagues across health and social care services to provide a whole systems, integrated and enhanced primary care approach to clients experiencing mental health issues.

Main duties of the job

The Population Health Nurse will have a caseload of patients with a focus on accessing support, care and treatment in a timely manner whether from their GP, secondary care or community services.

Work as an autonomous specialist nurse in population health.

Support other team members and colleagues in managing service users by providing training, coaching, supervision and advice support to develop core skills across the system.

Provide support to GP practices by liaising with C&I mental health services and other providers as required

Will have specific expertise in screening and identification of individuals that require further interventions in managing common physical health conditions/issues such as COPD, Diabetes and smoking cessation. Escalating and referring onto other services as appropriate.

Practice as a non-medical prescriber or will be working towards

Provide advice and support to practice General practitioners on psychiatric

Provide specialist training/supervision on the needs of people living with mental illness in the community.

Undertake a lead role in ensuring service user/carer experience is positive, services are responsive and accessible, and transitions between services are smooth and seamless.

As a Senior Specialist Clinician you will work directly with service users with varied mental health and comorbid physical health needs, actively supporting service users and providers to offer enhanced and integrated support to primary-secondary care patients to help manage fluctuating needs.

  • The Population Health Nurse role as agreed between the West and Central Camden PCN and community mental health Core Team, to support adults and older adults with complex mental health needs that are not suitable for IAPT provision.
  • Where a PCN engages one or more Mental Health Practitioners under the Additional Roles Reimbursement Scheme, the PCN must ensure that the Population Health Nurse provides the following functions depending on local context; As additional role practitioners would be ideally suited to help manage mental health patients and able to signpost where needed, and step up where needed into the Core Team.

The Populational Health Nurse will provide advice, support,

consultation, and liaison across the wider local health and social care

system.

The Population Health facilitation of onward access to mental and

physical health, well-being, and biopsychosocial interventions ---

- provision of brief psychological interventions, where qualified to do

so and where appropriate;

work closely with other PCN-based roles to help address the potential range of biopsychosocial needs of patients with mental health problems. This will include the PCNs MDT meetings, SMI annual physical health checks and Primrose Interventions. Conducting medication reviews for non-medical prescribers, and appropriately referring mental health patients for access to community-based support within the Core Teams such as Peer Coaching.

May operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate, subject to agreement on volumes and the mechanism of booking between the PCN and the provider. These may include frequent attendants into GP Practices who may not need medical intervention.
