This post comprises 6PA within the Trust personality disorder service (Intensive Team within the future Long Term Plan model) combined with 2PAs Camden core team liaison (Place based/ neighbourhood team within the future Long Term Plan model)
From March 2024, the personality disorder service will be located at Lowther Road Community Mental Health unit a newly built facility located in North Islington https://www.candi.nhs.uk/about-us/plans-redevelop-our-facilities/lowther-road
Main duties of the job
- Assist with diagnosis, medication reviews, management plans, risk assessments, crisis plans and self-help plans for patients with complex mental health problems.
- Work with Personality Disorder service to develop a collaborative care model for patients with disorders falling within the remit of the team.
- Provide clinical assessments of patients as necessary.
- Provide follow up as necessary for clients within teams.
- Support MDT in their diagnosis, management and treatment of patients through case discussions, joint consultations supervision and bespoke training.
- Play a key role in implementing improved linkage between 'core' community and 'intensive' teams within the Long Term Plan model.
