LOCUM CONSULTANT PSYCHIATRIST 10PA - General Adult Psychiatry

The post is for a one year fixed term, full time locum consultant psychiatrist and is available from June 22. The post-holder will be part of a small team providing primary care mental health support to GP practices in Kingston.

The team currently consists of a Consultant Psychiatrist, Locum specialty doctor, a B7 mental health nurse and admin support.

Main duties of the job

The aim of the service is to increase the number of people whose mental health support is appropriately managed within primary care.

19 GPs in Kingston have completed Diploma in Mental Health Care. The service provides support to GPs in managing patients with mental health problems which are too complex for local IAPT services but not meeting thresholds for secondary care. These patients cover a broad range of diagnostic groups.

Patients under secondary care mental health services are excluded from this service. The model is that of consultation, liaison, advice and assessment. The service does not offer traditional case management or carry a caseload. This is in order to maintain capacity of the service to respond quickly to GP referrals.

The post-holder will work as part of team with the aim of increasing the skill and confidence of GPs and other primary care staff to manage patients with mental health conditions in primary care. This will be achieved through a combination of consultation, liaison and advice to GPs, mental health assessments and reviews of clients and by developing collaborative approaches to case management through co-working and training.

Clinical and Management Duties

  • Work alongside MDT colleagues in order to assist GPs with diagnosis, medication reviews, management plans, risk assessments, crisis plans and self-help plans for patients with complex common mental health problems and stable severe mental health presentations who are being managed in primary care
  • Work with Primary Care teams to develop a collaborative care model for patients with physical and emotional difficulties, specifically for people with Medically Unexplained Symptoms (MUS) and Long Term Conditions (LTC)
  • Provide clinical assessments of patients as necessary.
  • Provide advice and support to primary care teams for patients who have been stepped down to primary care from secondary care community mental health services
  • Support GPs and practice staff in their management and treatment of patients through case discussions, joint consultations and bespoke training.
  • Play a key role in implementing improved linkage between primary and secondary care services
  • Work with the locality network to develop a triage system for potential referrals to secondary care
  • Maintain effective communication with GP, primary care staff, patients and their carers;
  • To provide clinical input, where necessary, to multi-disciplinary case conferencing around difficult patients being managed solely in primary care.
  • To engage with local service user participation models.
  • To work with local partners and commissioners in the development of a seamless referral pathway for GP referrals in Kingston
  • To provide clinical and professional accountability and to participate in management activities and governance processes related to the post or the wider service in which it is embedded.

  • Membership of Royal College of Psychiatrists or equivalent higher qualification in psychiatry
  • Section 12(2) and Approved Clinician status (or eligibility) - qualifies to act as a Responsible Clinician
  • Registered with Royal College of Psychiatrists CPD programme and in good standing

Desirable

  • Other relevant post-graduate qualification
  • Training up to date to be an approved trainer/clinical supervisor in line with GMC recognition and approval of trainers which is overseen by the LETB
  • Trained as an Educational Supervisor for doctors in training
  • Training as a Consultant mentor
