An exciting opportunity has arisen for band 5 practitioner to join the North Camden Crisis House

This post is open to registered clinicians (nurse, social worker or OT)

We will also strongly consider those who have worked at band 3 or 4 level who demonstrate exceptional experience:

a) 1 year + experience in a crisis house or crisis team setting as band 3 or 4

b) 2 years' experience in another acute setting who demonstrate and understanding of crisis house working

Ideally you would be educated to degree level or equivalent

We are seeking applicants who possess the skills and enthusiasm to take on the challenges of a changing, modern day mental health service.

We are a 24 hours service, 365 days per year providing mental health crisis care to our local adult residents. You will be required to be able to work any shift across this pattern including nights.

Our Crisis Houses all located close to good transport links. As we provide mental health services to a diverse and multi-cultural population we endeavour to reflect this in our workforce. Bilingual or multilingual applicants are strongly encouraged to consider applying.

This is an exciting time to join C&I Crisis pathway which includes working closely with crisis teams, working alongside the peer support network, provision of a crisis cafs and MIND

Main duties of the job

Working as a Band 5 in the crisis house will help you develop leadership skills, autonomous decision making, and dynamic risk assessment skills, as well as opportunities to progress in your career. You will have the opportunity to work and develop your skills as you move into autonomous decision making and assessments

You will be required to work shifts across the 24 / 7 day per week rota - including nights. Please consider this before making an application.

Assessments

Undertaking holistic assessments of people in crisis and deciding on collaborative treatment plans as an alternative to acute inpatient care working with colleagues and other services in the crisis pathway and in the Trust / outside agencies

You can access Trust training in house - for example CBT/DBT, de-escalation techniques, managing risks, brief supportive counselling skills to work towards shared treatment goals with the service user within their social system.

Supporting clients in the house in a therapeutic environment to live independent and fulfilling lives

Job responsibilities

- Reporting to the Team Manager or designated supervisor the postholder will undertake a range of necessary tasks, including taking and screening referrals, prioritising referrals, identifying appropriate action in processing referrals, carrying out assessments, developing and agreeing care plans with service users, implementing and evaluating care plans, and identifying and referring on to appropriate support services for service users ongoing care. This will involve delivering care and interventions appropriate to the post holders level of experience and competences without direct supervision.- To contribute to the multi-disciplinary team comprehensive, culturallyappropriate assessments to establish eligibility for health and social careservices.- To undertake continuous risk assessment and to contribute to effective risk management. At all times to act in accordance with trust policies in relation to risk assessment.- Recognising and valuing service users as individuals the post holder will ensure that the care provided respects equality and diversity that is supportive of service users and their carers. This will include encouraging service users to accept an optimum level of responsibility for their individual programme of care and with their consent, where appropriate, seek the cooperation of friends relatives and/or carers.- Utilising agreed frameworks and guidance such as NICE or NSF; evidence based best practice and or local policy and procedure, as well as a range of profession specific tools, the post holder will actively assess, plan anddetermine the care needs of service users.
