Job summary

The Community Team is a multidisciplinary team including social workers, psychiatric nurses, psychologists and consultant psychiatrists. The Team are all experienced professionals who have had specialist training in working with people who have complex needs that are characterised as personality disorder. The Team is part of a larger Personality Disorder Service, working alongside a psychological Therapies Team and Liaison Team to provide a specialist service delivering care that has been clinically shown to be effective in working with people who have this diagnosis.

The post holder will, along with the Clinical Lead and Service Manager, oversee the whole caseload of the team on a day to day operational basis. They will support the formulation of highly specialist care and treatment plans which provide highly skilled professional and specialist interventions / care to service users with complex mental health problems in line with best practice and service objectives. They will also be expected to hold a small, complex caseload.

Main duties of the job

The post holder, will lead and contribute to the management of a multi-disciplinary, multi-professional Mental Health care Team, is responsible for ensuring that the team understands and operates to the requirements of the service line operational policy and service line strategy and that safe, evidence based, effective, culturally competent care is delivered in a timely manner to service users.

In carrying out this role the post holder will make best use of the resources available to them in the monitoring of team and individual performance; ensuring that at all times best use is made of health and social care resources to assist recovery and re-enablement.

The post holder will lead on ensuring that service delivery focuses on the quality agenda and that dignity, privacy and respect is afforded at all times to service users. Team members are to be supported and developed to their potential and that new team members are to be comprehensively inducted and successfully integrated into the team.

About us

How we work:

The Team's approach is based on the Structured Clinical Management (SCM) model; a model that gives clinicians a framework to guide their work with service-users based on psychologically informed best practice. The essence of SCM is formulation based case management, including therapy, active crisis planning, and medication as an adjunct where appropriate. The Team works with people around social care as well as psychological needs. All of these elements are aimed at engaging people in a way that means they continue to be as independent as possible.

The SCM approach is not a rigid one, but it does give a clear understanding of the ethos that underpins our work working with people's strengths and abilities to build a life around their own goals.

The Community Team use the Care Programme Approach to plan interventions. In future this will be replaced with Dialog+.

The opportunity:

This is an exciting opportunity for an experienced registered professional, who has previous experience of working with this client group and leadership within a community mental health team, to develop these skills further and be part of a growing service that is dedicated to building positive futures for the staff team as well as the client group. You will be responsible, along with the Clinical & Operational Service Managers for the day to day running of the service and the ongoing evaluation of the work that we do.

Essential

  • Diploma or degree leading to professional Registered qualification in Mental Health or Substance Misuse Services
  • Current registration with relevant professional body
  • Evidence of recent and or ongoing post registration academic study relevant to practice

Desirable

  • Masters Degree in related field

SKILLS/ ABILITIES

Essential

  • Ability to communicate both in verbal and written form at a high level in a range of complex areas
  • Demonstrate well developed leadership skills with the ability to enthuse, motivate and involve team members
  • Evidence of effective workforce development and of using systems to ensure effective staff management (supervision, appraisal and ongoing performance management)
