Substance Misuse – Tier 4 Case Manager

Job summary

To support and enhance the social care of clients within Kingston Wellbeing Service; providing an assessment and interventions under the supervision of Deputy Service Manager, attending team meetings as appropriate, working independently according to a plan agreed with the Deputy Service Manager and within the overall framework of the team's policies and procedures. To assist in clinically related administration including, allocations and ensuring that relevant data is input on databases

Main duties of the job

To carry a caseload of case management clients, formulating care plans, identifying Tier 4 placements and applying/presenting cases for funding for placements as appropriate. In addition review and manage placements in Tier 4 treatment.

Job description

Job responsibilities

To be the main point of contact for providers and staff for referrals to the Case Management Service.

To ensure that all relevant information is provided on the referral, to check residency and be actively involved in the allocation of service users

To assist in the co-ordination and running of options group as required.

To work with other staff in the multi disciplinary team in the assessment and care provided to service users.

To attend and contribute to relevant multi disciplinary meetings.

To be flexible as travel to different sites within the service line will be essential

To work with budgetary constraints when identifying appropriate services ensuring the needs of service users are met.

To identify placements within the current Framework and to agree with Managers any placement that is made under services by exception

The postholder will be leading on the assessment, preparation, placement and case management of clients with complex issues in different settings

The postholder will lead on the co-ordination of seamless packages of care from residential to community

The postholder will jointly work with partners i.e. housing, safeguarding, ASC, criminal justice to ensure the very best outcomes for service users accessing Tier 4

The postholder will be responsible for completing their own administration and reporting in relation to Tier 4 including quarterly activity/outcome reports

The postholder will lead on the completion and presentation of high quality/comprehensive panel applications and/or reviews for detox and rehab
