Proactive Care Team Specialist Physiotherapist

Job summary

The awarding winning proactive care team provides anticipatory care to adults with multiple long-term conditions in a defined area, through a combination of referral and case finding. The team seek to reduce inequalities in care and prevent unnecessary hospital admission and maximise patients potential through a multidisciplinary individualised approach.

The post holder will work as part of a multidisciplinary team with blurred boundaries to reduce risk of admission and maximise patients' potential. As a Specialist Physiotherapist you will be required to undertake assessment, evaluation and treatment of complex patients. Working in conjunction with the wider health, social and voluntary sector to provide evidence-based practice.

Main duties of the job

The Specialist Physiotherapist post is predominantly a clinical role with a split of 90% clinical and 10% non-clinical work.

The post holder will be responsible for their own caseload within their speciality area as an autonomous practitioner.

As a specialist physiotherapist they will assess, plan, deliver and evaluate patient centred Physiotherapy programmes of care to a specialist level ensuring that they practice within their sphere of competence and knowledge.

Knowledge and skills will be used to develop competencies and clinical knowledge with patients, carers, colleagues, students and the wider multidisciplinary team. As a team member they will also support members of the wider multidisciplinary team deliver high quality community care.

The post holder will work to professional and HCPC standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.
